Our office's COVID-19 safety protocols are still in place to ensure the safety of our patients and staff.  We appreciate your cooperation and compliance with the procedures below:

Masks are REQUIRED at ALL times when you are inside our office.

APPOINTMENTS

1.  AT HOME: Before you leave for your appointment, please check your temperature at home.  If you have a temperature of 100.4 or higher, are experiencing cough, shortness of breath, or other symptoms, please inform us as soon as possible.  We will be happy to reschedule your appointment for when you are feeling better.  A full list of COVID-19 symptoms as outlined by the Centers for Disease Control can be found at https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html .

For new patients, please print and complete the New Patient Registration Forms at home before your appointment.  To keep the process as contactless as possible, please email your completed forms to [email protected]
or fax to (703) 823-1572.
If you don't have access to a printer or fax machine, arrive to our office at least 20 minutes before your appointment time to complete your forms in your car/outside of our office.

2.  ARRIVAL: When you arrive at our building, DO NOT ENTER OUR OFFICE.  Instead, wait inside your car or outside, and call our office at (703) 823-2357 to let our staff know you have arrived.  If it is a hot day, please leave your car running with the air conditioner on for your health and safety.

3.  YOUR APPOINTMENT: When the doctor is ready to see you, a staff member will meet you at your car to take your temperature, ask you a few screening questions, and escort you directly to the treatment room.  WEAR A MASK and maintain at least 6 feet of physical distancing, when possible, inside our office.  If you do not have adequate face covering, we will need to reschedule your appointment.  Payments and follow-up scheduling can be processed at the front desk, but we ask that you stand at the yellow taped area to maintain social distancing.

Thank you very much for following these procedures.  If you have any questions, please feel free to contact us.  It is our goal to provide you a safe environment for your healthcare.

If you need to reschedule or cancel your appointment, please let us know no later than 24 hours before your scheduled appointment by calling our Alexandria office at (703) 823-2357.
